Business Interruption Insurance Questionnaire

Please complete the questionnaire below

1. What is the nature of your business and what are your usual business activities?

2. Do you have a physical business premises from which you operate?

3. Did you close your business as a result of the COVID-19 pandemic? If so, on what date did you close?

4. Why did you close your business? i.e. were you directed to close by a public authority / regulatory body / any other authority, or for some other reason?

5. When did you reopen your business?

6. Between the date on which you closed your business and the date on which you reopened (“the Closure Period”), did you reopen intermittently between these dates for any period? If so, how long for and to what extent were you able to carry out your usual business activities (as you have defined above)? Please set out a brief timeline.

7. When you reopened your business, did you reopen fully? i.e. did you fully resume your usual business activities?

8. During the Closure Period (between the date on which you closed your business and the date on which you reopened), whilst your business premises were physically closed, did you continue to carry out your usual business activities by an alternative means (e.g. selling online instead of in store)?

9. Whilst your business premises were physically closed, did you change the nature of your business activities (e.g. selling takeaway food instead of dine-in food)?

10. If you were able to continue trading whilst your physical premises were closed, albeit in a different manner, to what extent do you estimate your business turnover to have been impacted by the shift in the manner in which you trade?

11. Are there any factors other than COVID-19 (and the related restrictions) that would have meaningfully affected your business’ turnover during the Closure Period and the Indemnity Period? For example, a retail business which had opened a second shop shortly before the first lockdown in March 2020.

12. After you reopened your business, did your cost of working increase as a result of COVID-19 and the related restrictions? If so, please outline the reasons why. For example, by purchasing PPE equipment, using disposable rather than reusable items (such as plates/cutlery/cups) or employing more staff?

13. What communications did you have with your insurer during the Closure Period and during the Indemnity Period? Please summarise here.
