If you would like to set up a Direct Debit (DD) with us, please download the form below.

Please print, complete, and sign the form (must be a wet signature not digital) then return by post to:

Accounts Team, Milsted Langdon, Winchester House, Deane Gate Avenue, Taunton, TA1 2UH.

Please ensure the following details are completed:

  1. Name of the account holder
  2. Account number
  3. Sort code
  4. Bank name and address of bank
  5. Signature
  6. Date

It typically takes about a month for the DD to be set up by the banks before we can make a claim.

Payment collection under our DD scheme is done the last working day following month after the Request For Payment (RFP) date. Should you have any queries about your RFP, please let us know asap and collection would be put on hold until an agreement is reached.
